Thank you for choosing Manifest to host your special event. We believe beauty services and cocktails make for one amazing event! We appreciate you taking the time to make us aware of all the pertinent details so we can make sure your group has the best EXPERIENCE possible. We are happy to help you in any way. The purpose of this agreement is to confirm the names and services of everyone in your group. All information must be completed and returned in order to see availability

*PLEASE READ*

*Any services not canceled 2 weeks prior to the event or that no-show the day of the event will be charged full price of the services reserved. *No changes can be made to the services the day of the event *Please have all members of your group arrive together and on time. Arriving more than 15 minutes late can result in the cancellation of your group. * All changes or add on requests must be made 2 weeks in advance and in writing through Email. Unfortunately we can not make changes over the phone. If we can accommodate last minute changes the day of the event we are happy to do so, but can not guarantee availability. *All services will be charged to the card below. A 20% Gratuity is added for all groups of 6 or more. *Food of your choice is permitted to be brought in, as we serve no food in house. No paper products are supplied *No outside alcohol is permitted *Decorations of your choice may be used and set up 30 minutes prior to the start of your services. (confetti is not permitted) Please indicate if any members of your group need to be taken first for services *We have 3 different pedicures and multiple manicure services. Descriptions and pricing can be found on our website www.manifestcle.com Please list the specific service you are requesting as each service requires different timing. *Add gel removal if needed Manifest specializes in natural nail care. The health and integrity of your nails is our main priority. We do not have the tools or remove any kind of acrylic, hard gel or dip. We ask that all guests getting serviced have these products removed prior to their reservation
